Are More Graft Numbers Better?

When it comes to hair transplantation, it’s easy to assume that more grafts must mean a better result.


Up to a point, perhaps. But bigger numbers don’t automatically mean better surgery — and attempting to transplant excessive numbers of grafts in a single procedure can increase risk.


One of the most important considerations is graft survival and quality of growth. It isn’t enough for a graft simply to be transplanted. It needs to survive and produce the hairs it contained. For example, if a three-hair graft survives but ultimately produces only one hair, the patient has lost around 67% of the potential hair from that graft.

Very large procedures can also increase the risks associated with prolonged surgery and extensive treatment of the scalp. Potential complications include poor graft growth, donor or recipient-site necrosis, and local anaesthetic toxicity. In severe cases, local anaesthetic toxicity can cause seizures, cardiac complications and can be life-threatening.

There is another consideration that is sometimes overlooked: what happens if the transplant doesn’t grow as expected?


A responsible surgeon should always leave a “lifeboat” — sufficient donor hair and a realistic backup plan should there be partial or significant graft failure. Using too much of the donor supply in one operation can severely limit the ability to correct a disappointing result.


And hair loss doesn’t necessarily stop after a transplant. A patient may want further treatment as they get older. Your donor hair is finite and needs to last a lifetime.
